Children’s / Targeted Case Management

The Children’s Case Management Program of Independence Association offers caring, compassionate support to children age three through age twenty who have been diagnosed with a Developmental Disability, Autism or Mental Health needs.

We offer:

  • Family focused treatment planning based on strengths, needs, and preferences
  • Coordination of services and supports that will evolve to meet the child’s emerging social, educational, and therapeutic needs
  • Advocacy, networking, and resource development support to meet all identified needs of both the child and family.
  • Providing continuous supportive contact with other involved service providers.
  • Transition and discharge planning
  • We serve Cumberland, Sagadahoc, Lincoln, Androscoggin and Kennebec Counties.
Our team of professional case managers will work with the child and family in their home to ensure that services are coordinated in a place that is comfortable and at a time that is convenient for the family. Home visits ensure a level of sensitivity and caring when receiving support. Case management is youth and family driven to achieve the highest level of success. We identify the strengths of each child and family and use these strengths to successfully meet the identified needs. The Children's Case Managers coordinate the supports for the child, to best meet the child’s emerging social, educational and therapeutic needs through creative, strength-based service planning. 

Referral Process

Referrals can be made by parents, providers, and other professionals working directly with the child by contacting us at 207-725-4371 and ask to speak to the Program Manager of the Children’s Case Management program.
