Children’s / Targeted Case Management

The Children’s Case Management Program of Independence Association offers caring, compassionate support to children age three through age twenty who have been diagnosed with a Developmental Disability, Autism or Mental Health needs. We serve Cumberland, Sagadahoc, Lincoln, Androscoggin and Kennebec Counties

We offer:

  • Family focused treatment planning based on strengths, needs, and preferences
  • Coordination of services and supports that will evolve to meet the child’s emerging social, educational, and therapeutic needs
  • Advocacy, networking, and resource development support to meet all identified needs of both the child and family.
  • Providing continuous supportive contact with other involved service providers.
  • Transition and discharge planning
Our professional case managers work with the child and family in their home for comfortable, convenient and sensitive service coordination. We identify the strengths of each child and family and use these strengths to successfully meet the goals identified by the child and family. Children's Case Managers coordinate supports to best meet the child’s emerging social, educational and therapeutic needs. 

Referral Process

Referrals can be made by parents, providers, and other professionals working directly with the child by contacting us at 207-725-4371 and ask to speak to the Program Manager of the Children’s Case Management program.
