Adult Case Management
Independence Association provides comprehensive case management services to Adults with Disabilities. We accomplish this goal by developing Person Centered Plans with meaningful and attainable goals based on the individual strengths, needs and desires of the person we are supporting. Coordination of necessary services, advocacy, referral and monitoring of services to ensure success are just a few of the ways that we provide our support services. Home, program and community visits are an essential part of the service. We strive to develop a system of natural supports to help people achieve and maintain independence.
Quality of life
Quality of life is a key focus of our comprehensive case management program. Our case managers are a highly skilled team of professionals who work with people to meet their desires while focusing on our mission of supporting people to lead a full and inclusive life in their chosen community.
Representative payee services are available to recipients of our Adult Community Case Management Program.
Eligibility for case management service is determined by the Office of Aging and Disability Services, division of the Department of Health and Human Services. Referrals may be made directly to the agency by calling 207-725-4371 and ask to speak with the Program Manager of the Adult Case Management program.
